•  Marriage certificate
•  Husband’s birth certificate in Arabic
•  Wife’s birth certificate

* In the case of a change of the wife’s surname: 

- Certificate of change of surname

THE BIRTH TRANSCRIPTION FILE CONSISTS OF:

•  The birth transcription application form
•  Child’s birth certificate
•  Father’s birth certificate in Arabic
•  Mother’s birth certificate

* In the case of a change of the mother’s surname: 

- Certificate of change of surname
